At Aurora Functional Healthcare we do our best to help you save money. If your insurance provider covers conventional range test, we do not require you to pay for tests that are covered by your insurance. Our Functional Laboratory tests are not covered by insurance. Hence, this is an out-of-pocket expense. We accept FSA/HSA cards helping reduce cost.

In general, Medicare PPO plans offer better coverage for out-of-network care than HMO plans, but this is not always the case.

Payment is due prior to your appointment for both tele-health and in person visits. Due to high quality of service given. All sales are final. You will have an option to rebook at your convenience within all USA states. Due to high patient volume, this gives the practitioner enough time to prepare for your visit and review healthcare documents and secure your seat. In an efforts to dispute your booked appointment with your credit credit card provider after you have consulted with the practitioner you will not be granted a refund. As we do not encourage theft in our facility as this is a criminal act and will be reported to the local police department. Any questions you may have give us a call prior to booking your appointment. A representative will address your questions and concerns.

At Aurora Functional Health, we value transparency and want every client to understand how insurance billing and unpaid balances are handled within our practice. The following policy outlines our procedures regarding insurance claims, write-offs, and patient responsibility.

1. Insurance Billing

As a courtesy, we may submit claims to insurance providers when applicable. Submission of a claim does not guarantee payment. Coverage, reimbursement, and allowable amounts are determined solely by the insurance company.

2. Patient Responsibility

Clients are responsible for:

• Any portion not covered by insurance
• Deductibles, copays, and coinsurance
• Services not eligible for reimbursement
• Balances remaining after insurance processes the claim

If insurance denies or does not fully pay a claim, the remaining balance becomes the patient’s responsibility.

3. Write-Offs

We do NOT write off balances solely because insurance did not pay.

Unpaid or denied charges are not eligible for discretionary write-offs unless:

• A contractual adjustment is mandated by the insurance payer, or
• An internal billing error occurred.

4. Non-Payment of Claims

If UnitedHealthcare or any insurer does not pay a claim, the following steps occur:

• We recheck the claim for accuracy
• We perform one follow-up appeal or inquiry

If the claim remains unpaid, the balance is transferred to the client

5. Financial Hardship Consideration

We offer hardship review for clients experiencing genuine financial difficulty. Any hardship adjustment requires:

• A formal request
• Verification documentation
• Administrative review and approval

Hardship adjustments are not automatic and are determined on a case-by-case basis.

6. Tax & Accounting Compliance

As a healthcare provider, we follow IRS guidelines regarding write-offs and bad debt.

Balances that are uncollectible may be written off internally for bookkeeping, but this does not guarantee or imply tax deductibility.

7. Agreement of Understanding

By receiving services at Aurora Functional Health, clients acknowledge and agree to abide by this Insurance Unpaid Balance Policy.
